This can be done in two ways:<\/p>\n<ul>\n<li>The principal resident gives permission during the online declaration (log in with DigiD).<\/li>\n<li>During the declaration, you attach the consent form and a copy of an identity document of the main occupant.<\/li>\n<\/ul>\n\t\t\t<\/div>\n\t\t  <\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"accordion-item\">\n\t\t  <h2 class=\"accordion-header\" id=\"heading-u1-i2\">\n\t\t\t<button class=\"accordion-button collapsed\" type=\"button\" data-bs-toggle=\"collapse\" data-bs-target=\"#collapse-u1-i2\" aria-expanded=\"false\" aria-controls=\"collapse-u1-i2\">\n\t\t\t  <span class=\"accordion-toggle\"><\/span>Passing on\t\t\t<\/button>\n\t\t  <\/h2>\n\t\t  <div id=\"collapse-u1-i2\" class=\"accordion-collapse collapse\" aria-labelledby=\"heading-u1-i2\">\n\t\t\t<div class=\"accordion-body\">\n\t\t\t  <p>You can notify us of a move in three ways:<\/p>\n<h3>1. Online<\/h3>\n<p>Report your move online using the \u2018Report\u2019 button at the top of this page.<\/p>\n<p><em>Move in with someone<br \/>\n<\/em>In this case, a signed consent (authorisation) from the main resident is required. You have a choice of how you want to provide this.<\/p>\n<ul>\n<li>By having the main resident log in with DigiD during your online application<\/li>\n<li>By providing a copy of the identity document and the completed and signed\u00a0<a href=\"\/wp-content\/uploads\/2026\/04\/toestemmingsverklaring-20260326-wcag.pdf\">consent form<\/a>\u00a0(pdf, 745 kB) of the main resident to be uploaded.<\/li>\n<\/ul>\n<p><em>Moving to an institution<br \/>\n<\/em>In this case, the completed\u00a0<a href=\"https:\/\/www.apeldoorn.nl\/wp-content\/uploads\/2026\/05\/toestemmingsverklaring-20260326-wcag.pdf\">consent form<\/a>\u00a0(pdf, 745 kB) showing the stamp of the (healthcare) institution is necessary. You do not need to upload a copy of the proof of identity of the head of the healthcare institution.<\/p>\n<h3>2. Desk<\/h3>\n<p><a href=\"\/beheer\/beslisbomen\/bb-afspraak\/afspraak-verhuizen-binnen-nederland\/\">Make an appointment online<\/a>\u00a0To communicate your move.<\/p>\n<p>Take to appointment:<\/p>\n<ul>\n<li>A valid identity document<\/li>\n<li>Are you moving in with someone? If so, please take the completed\u00a0<a href=\"https:\/\/www.apeldoorn.nl\/wp-content\/uploads\/2026\/05\/toestemmingsverklaring-20260326-wcag.pdf\">consent form<\/a>\u00a0(pdf, 745 kB) along with the stamp of the (healthcare) institution.<\/li>\n<\/ul>\n<h3>3.
