You can also use this form to revoke an authorisation you have previously granted.<\/p>\n<p class=\"styles_text__Tljo9\">The authorisation is valid for 1 year. You may, of course, withdraw the authorisation at any time. Please complete the form in full.<\/p>\n<\/div>\n<\/div>\n<div>\n<div>\n<div>\n<p>If you do not have a DigiD, you can download the authorisation form below and, by appointment, hand it in in person at the Town Hall.<br \/>\n<strong>Please note!<\/strong> To hand in the authorisation, you must make an appointment by telephone.
