During the appointment, you will need:

• How many appointments do you have, or are you due to have, at your healthcare centre or hospital?
• How often do you visit a detention centre or prison?

On the next page, you can add multiple activities, but this is not necessary. If you wish to create an appointment with multiple cost types, one cost type (activity) is sufficient.

You cannot book an appointment yourself for other charges

If the costs for which you wish to apply for special assistance are not listed in the drop-down menu, please contact us by telephone on 14 055.

To avoid you making an unnecessary trip to the office, please wait for an appointment. We would like to discuss your request for assistance with you over the phone first. During the phone call, we will work with you to see whether you might be eligible for special assistance or if there is another solution for you. If necessary, we can still arrange an appointment for you at the special assistance counter during the phone call.
