Are you no longer able to live at home due to mental health and/or psychosocial problems? Please discuss your needs with a member of staff at Samen055. You can call 055 580 11 40.
We will briefly discuss your enquiry to see if we can help you straight away with information or advice. If this isn’t possible, we’ll take your details and note your enquiry, and a counsellor will ring you within one to three working days at the latest to discuss your enquiry. If necessary, the counsellor will arrange an appointment with you for a visit to your home.
The role of the local authority
As the central local authority, Apeldoorn Council is responsible for supported housing in this region. We assess applications from residents of the municipalities of Apeldoorn, Voorst, Epe, Brummen or Heerde. It is also possible for a person seeking supported housing to have a demonstrable, strong connection to this region.
Are you from a different region? If so, please fill in the Assessment form for national accessibility in the East Veluwe region In.
Criteria for supported housing
If you are assessed as needing supported housing, we will look for a suitable place for you to live. This could be a room in a care home or a community-based housing scheme. You are free to contact a provider yourself, but we can also do this on your behalf. You may be placed on a waiting list if there are no places available.
Whether someone is eligible for supported housing depends on their specific circumstances. What difficulties do they face? How have they lived up to now? Is support at home a suitable solution? Only if support at home is insufficient will the local authority issue a referral for supported housing.
The person in question is a Dutch national or is legally resident in the Netherlands.
If you use support or an aid provided under the Wmo, you will almost always have to pay a personal contribution. It does not matter whether you use this support or aid in kind receives, or in the form of a personal budget. You can find more information on the page personal contribution.
You do not need to do anything if you no longer require supported housing.
Please let us know if you would like to extend your support. To do so, please fill in the form together with your support worker the notification form from Samen055. Please do this 6 to 8 weeks before the end date of your support period. A Wmo adviser will call you within one working day and no later than three working days so that they can discuss your support request with you. If necessary, the Wmo adviser will arrange an appointment with you for a visit to your home.
The appointment at your home will last approximately one hour to one and a half hours. We would ask you to bear this in mind when making other arrangements
