All Dutch and foreign travel documents (passports and identity cards) registered in your name. Even if they are no longer valid
On 1st application or missing: a valid driving licence if you have one.
A good-looking colour passport photo not more than six months old at the time of application.
Means of payment (prefer debit cards to cash).
If you apply for a document for your child, he/she must come with you in person.
Younger than 12? Signed declaration of consent and (copy) valid ID from both parents/guardians. This consent can also be given online in advance via our website www.apeldoorn.nl/toestemmingsformulier. You will need DigiD for this. Is online not possible? At www.apeldoorn.nl/reisdocumenten you will find a consent form that you can fill in manually. You will need (a copy of) the valid identity document of the authorised representative.
