During the appointment, you will need:

All travel documents in your possession, regardless of whether they are still valid
Good-looking colour passport photo not older than 6 months
NoteDid you take your passport photo using an app? If so, there’s a good chance the photo has been automatically edited. We are not permitted to accept edited passport photos.

Means of payment (prefer debit cards to cash)
If you are applying for a document on behalf of your child, he or she must come with you in person
Under 18 years of age: a signed consent form and valid proof of identity for both parents/guardians

Make appointment

During the appointment, you will need:

All Dutch and foreign travel documents (passports and identity cards) registered in your name. Even if they are no longer valid
On 1st application or missing: a valid driving licence if you have one.
A good-looking colour passport photo not more than six months old at the time of application.

Means of payment (prefer debit cards to cash).
If you apply for a document for your child, he/she must come with you in person.
Under 18? A signed consent form and a copy of a valid ID document for both parents/guardians. This consent can also be given in advance online via our website www.apeldoorn.nl/toestemmingsformulier. You will need a DigiD for this. If you are unable to do this online, you can also complete a declaration of consent by hand. To do so, you will need a valid form of ID (or a copy) belonging to the person with parental authority. Go to the Consent form (PDF, 75.6 kB).
