During the appointment, you will need:

• Valid proof of identity (passport, identity card or driving licence)
• When registering the first child from a relationship, the mother must be present to discuss the choice of surname. She must also bring valid proof of identity
• When recognising a second or subsequent child from a relationship, written consent from the mother, together with a copy of her valid identity document, is sufficient
