You are registered as a resident of the municipality of Apeldoorn.
A Certificate of Life is intended for use by Dutch authorities. For foreign authorities, you will need a Certificate of Life required.
There are two ways to apply for a Certificate of Life:
Online
Apply for the certificate online.
The certificate will be sent by post to your home address within three working days.
Desk
Book an appointment online to apply for the certificate.
Take to appointment:
- A valid identity document
- The letter from your pension fund when you request proof of this
You will be given the certificate straight away.
Authorisation
You can authorise someone else to apply for and collect a life certificate on your behalf. To do so, please use the authorisation form (pdf, 160 kB). The authorisation must not be a scan or a copy. Your original signature is required for the application. You cannot, therefore, email the form to the person who is submitting the application on your behalf.

If your pension fund requests this proof, please enclose it with your application; in that case, the application is free of charge. If your pension insurance (life insurance) requests this proof, you will have to bear the costs yourself.
