You can use this form to authorise someone (a family member or acquaintance) to discuss your affairs relating to work, income, debts and the minimum income policy with Apeldoorn Council over the telephone. You can also use this form to revoke an authorisation you have previously granted.
The authorisation is valid for 1 year. You may, of course, withdraw the authorisation at any time. Please complete the form in full.
If you do not have a DigiD, you can download the authorisation form below and, by appointment, hand it in in person at the Town Hall.
Please note! To hand in the authorisation, you must make an appointment by telephone. You can do this by calling 055 – 580 11 40 (Monday to Friday from 8.30 am to 5.00 pm).
