At the community meeting places, there is also day care offered.

Do you need help filling your day? On the community meeting places In the neighbourhoods, you can take part every day in various activities. Day care is also available here.

You only pay for your food and drink and, if applicable, for any activities. You do not pay a personal contribution because you do not need a care assessment from the local authority.

The community centres are open to all residents of the municipality of Apeldoorn. However, the day-time activities at the community centres are primarily intended for, and best suited to, residents with a low level of support needs. In practical terms, this means that:

• if you need more intensive support that cannot be provided at a community centre. For example, because the group size (an average of 10 visitors) is not suitable for the level of support required, or because the support available at a standard meeting place is insufficient due to cognitive or mental health issues. In such cases, a bespoke support assessment must be requested;
• If you have a WLZ assessment, you will be referred to a WLZ day centre.

In practical terms, this means that you may sometimes start at a basic meeting point, but will need to “move” to a different location or provider if your support needs become more complex. Or if you are no longer receiving support under the WMO but under the WLZ.

Do the activities and daily programme at the (Basic) meeting places no longer suit you? Please discuss your request for help with a member of staff at Samen055. Here, you and the staff member will explore the options for daytime activities. If necessary, you can be assessed for support.

At the day centre, you will have a support worker who will work with you on your personal development goals. You will talk to each other regularly about how things are going.

Would you like to play your part and take steps towards finding work? If so, you can take part in Apeldoorn Gets Involved. In a suitable environment, you’ll work towards your goals and receive guidance to help you grow.

What happens if you are assessed as needing care?

If you have been assessed as needing care, there are two ways to get help.

• Care in kind: You will receive assistance from one of the providers with whom the local authority has a contract. You can choose who you would like to provide your care, and the local authority will handle the arrangements.
• Personal budget (pgb): You will receive a sum of money which you can use to find the support that suits you best.

Is the assessment for the day care programme no longer appropriate?

Are you currently attending a day centre, but is your care assessment no longer accurate – for example, because you are attending more often, or need to attend less often? Discuss your situation with a member of staff at Samen055. Here, you can discuss the options for support with the staff member. There are day centres where professionals offer additional support.

Do you pay a personal contribution for day care under the Wmo?

Yes, you (almost always) pay a personal contribution: the subscription fee.
If you have been referred to Apeldoorn Werkt Mee, you will not have to pay any personal contribution for this.

Is your referral for day care due to expire soon?

Do you currently have a referral for day care, and is it due to expire soon? Discuss with your care worker whether you still need day care.

You do not need to do anything if you no longer require day care.

You can book an appointment yourself.

We would be happy to discuss your application for the extension of day care with you. You can use the ‘Book an appointment’ button at the top of the page to book an appointment with a neighbourhood adviser yourself.

The meeting with the neighbourhood adviser will last approximately one hour to one and a half hours. Please bear this in mind when making other appointments.

Is your referral for Apeldoorn Werkt Mee coming to an end?

Your case worker will then arrange an appointment to discuss the extension.
