  • if you are unable to reach a solution with a healthcare professional or a local authority staff member
  • if you are worried about your own situation or that of someone else
  • if you feel that something isn’t quite right

Once you have reported the issue, we will assess what needs to be done. Sometimes we can help you straight away. Sometimes we will investigate the situation or refer it to the appropriate organisation.
