Deze functie als Wijkconsulent Zorg en Ondersteuning (28 – 36 uur) is jouw volgende stap als je wilt bijdragen aan de ontwikkeling van inwoners, onze dienstverlening én jezelf.

Sinds januari 2024 werken we binnen Samen055 met vijf wijkteams. Elk wijkteam bestaat uit professionals met vijf verschillende profielen: Bestaanszekerheid, Zorg & Ondersteuning, Opvoeden & Opgroeien, Individuele Ondersteuning en Intensieve Regie. Bij het wijkteam kunnen inwoners terecht met vragen op alle levensdomeinen. De professionals in het wijkteam werken als wijkconsulent binnen één van de verschillende profielen. Jij wordt wijkconsulent Zorg & Ondersteuning.

Bij Samen055 vinden we dat iedereen recht heeft op een sterke basis. Inwoners die een hulpvraag hebben, help jij hun weg te vinden, hun netwerk te versterken en oplossingen te creëren die écht werken. Jij behoudt het overzicht over de verschillende leefgebieden en disciplines en zorgt ervoor dat de juiste ondersteuning wordt ingezet. Je hebt een regierol en je werkt actief aan het vergroten van de zelfredzaamheid van inwoners.

Here’s what you’ll be doing
Nederland streeft ernaar dat mensen zo lang mogelijk thuis kunnen blijven wonen. Dit vraagt om maatwerk, waarbij het sociale netwerk en de eigen kracht van inwoners optimaal worden benut om de zelfredzaamheid van de inwoners te vergroten. Als wijkconsulent Zorg & Ondersteuning doe je integraal onderzoek naar de mogelijkheden en beperkingen van de inwoner en diens omgeving. De inwoner komt met een hulpvraag en jij weet de achterliggende vraag te achterhalen. Je voert regie op het proces en zorgt ervoor dat andere wijkconsulenten tijdig worden betrokken wanneer dat nodig is. De behoefte van de inwoner staat centraal; wet- en regelgeving vormen hierbij het kader, maar niet het uitgangspunt. Je weet hoe je (maatwerk)voorzieningen op de juiste wijze inzet. Je bent gericht op wat de inwoner echt verder helpt.

You take a broader view than just the Wmo; you have an understanding of all relevant areas of life and the situation in which the resident and their household find themselves (for example, financial security). You have sufficient knowledge and experience of working with different target groups (e.g. people with acquired brain injury, autism, mental health conditions or intellectual disabilities) and can easily liaise with them, their support network or the professionals involved.

In practical terms, this means that you:

• Conducts interviews in residents’ homes
• Hulpvragen (breed) in kaart brengt
• Inzet op eigen kracht van inwoners, netwerk rondom de inwoner en het voorliggende veld
• Beoordeelt of er Wmo-voorzieningen toegekend mogen worden of dat er andere vormen van ondersteuning passend zijn
• Residents are referred to services within the local authority or to the local authority’s partners.

Als wijkconsulent Zorg & Ondersteuning ga je aan de slag met:
De wijkconsulent kent de mogelijkheden in de wijk, de sociale basis en stimuleert en activeert de inwoner gebruik te maken van het voorliggende veld. Samen met de professionals in het netwerk organiseer je verder ondersteuning op alle levensdomeinen voor inwoners die dat (tijdelijk) nodig hebben. Je werkt hierbij samen met collega’s van Samen055, voorzieningen in de wijk, aanbieders en andere professionals.  Naast je regierol ben je ook beschikbaar voor consultatie. Je denkt mee bij vraagstukken, adviseert collega’s en sluit waar nodig aan bij huisbezoeken.

Specifically, you’ll be working on:

• Thoroughly exploring and clarifying residents’ requests for assistance
• Providing information and advice (promotion and prevention)
• Drawing up a comprehensive plan
• Identifying, coordinating and evaluating
• Managing a caseload, coordinating and evaluating
• Where necessary, assessing the need for bespoke support measures 

Who are you?

• You’re quick on the uptake, take the initiative and aren’t the type to sit back and wait;

• You are not afraid to set boundaries and say “no” when something is not possible or appropriate, and you can explain this clearly and in a way that is easy to understand.
• You know how to manage expectations realistically and can guide residents when a solution they would like to see does not prove feasible.
• You remain empathetic and professional, even in situations where emotions or resistance come into play.

• You have proven experience in conducting interviews and are able to analyse a situation quickly
• You have the ability to think critically and do not shy away from engaging with residents, even when this may be challenging or confrontational.

In addition, you will have:

• Je hebt een afgeronde hbo-opleiding
• Rijbewijs B
• Je hebt minimaal 2 jaar werkervaring binnen de Wmo
• You have the necessary knowledge of legislation and regulations relating to the Wmo, the social security system and other financial assistance schemes 
• It is an advantage if you are familiar with the social services in Apeldoorn or can quickly familiarise yourself with them 
•  You have strong communication skills, are sociable, can cope well with stress, and possess empathy and a strong sense of responsibility. You are also flexible and able to work both independently and as part of a team.

Dit bieden we jou:

• Een dienstverband voor 28- 36 uur per week, in eerste instantie voor de duur van één jaar;
• A salary of at least €3,206 gross per month (entry-level scale 8) and up to €5,033 (functional scale 9) for a 36-hour working week. The actual salary depends on your experience and qualifications.
• An Individual Choice Budget (IKB) of 17,45% on top of your salary. You can use this, for example, for:
• Extra income by having it paid out. 
• Buying extra holiday hours.
• A good work-life balance, with the option to save up leave and flexible working hours

• 201.6 holiday hours for a full-time position (36 hours)

• Can you work (partly) from home in your role? If so, you will receive a home-working allowance.
• Do you travel to work by public transport? If so, you’ll be fully reimbursed. Do you travel by car, bike or on foot? If so, you’ll receive an allowance for this.
