Here’s what you’ll be doing
As a Wmo consultant within the Housing, Wheelchairs & Transport cluster, you are a specialist in the field of home adaptations, wheelchair and mobility aids, and transport services. You will handle complex requests for assistance, taking into account not only the resident’s personal circumstances but also the technical, practical and financial feasibility of solutions.
You assess the capabilities and limitations of residents and their living environment, and align your work with the neighbourhood team’s integrated plan. You encourage residents to make the most of their own strengths and networks. You are familiar with Apeldoorn’s social services directory and make effective use of (tailored) support services when necessary. In doing so, you regularly collaborate with service providers, medical advisers and other partners in the care chain.
