Since January 2024, we have been working with five neighbourhood teams within Samen055. Each neighbourhood team consists of professionals with five different specialisms: Basic Needs, Care & Support, Parenting & Child Development, Individual Support and Intensive Coordination.

Here’s what you’ll be doing
Within Samen055, the ‘Intensive Coordination’ profile provides the expertise required for cases involving safety risks and where intensive coordination is needed. Examples include:
Depending on the situation, support may be requested, but may also be offered unsolicited if there is cause to do so. In the case of adults who display behaviour that is difficult to understand, an intervention-based approach may be adopted. Within this role, you will deal with all sorts of different households and families. Due to the high number of cases involving young people, the Intensive Coordination team comprises, in part, staff from the Centre for Youth and Family (CJG).
Intensive coordination acts as the link between the protection chain (Veilig Thuis, certified institutions, the Child Protection Board and the ‘Toekomsttuin’) and the safety chain (Care and Safety Centre, probation service, police, etc.)
