Neighbourhood Consultant, Intensive Coordination

Since January 2024, we have been working with five neighbourhood teams within Samen055. Each neighbourhood team consists of professionals with five different specialisms: Basic Needs, Care & Support, Parenting & Child Development, Individual Support and Intensive Coordination.

As a Neighbourhood Consultant for Intensive Coordination at Samen055, you are the linchpin that brings people, organisations and solutions together. In this way, you work every day towards a society in which everyone can participate.

This vacancy from Neighbourhood Consultant – Intensive Coordination (32–36 hours) is your next step if you:

• no longer wants to say ‘this isn’t my responsibility’. You work across different sectors, examine the issues in all areas of life, assess what is needed and take the lead in the resident’s care plan
• wants to exercise close control over complex security cases
• wants to play a unifying role. You will ensure that you bring together various internal and external parties to work on a single plan. Where necessary, you will arrange access to support services under the Social Support Act (WMO)

Here’s what you’ll be doing
Within Samen055, the ‘Intensive Coordination’ profile provides the expertise required for cases involving safety risks and where intensive coordination is needed. Examples include:

• Managing complex cases.
• Co-ordinating support from various organisations and disciplines.
• Strengthening residents’ self-reliance.
• Encouraging self-reliance and the social network.
• Drawing up, monitoring and revising support plans.
• Bringing together residents, partners in the care chain and care providers.
• Identifying risks and bottlenecks and taking action in response.

Depending on the situation, support may be requested, but may also be offered unsolicited if there is cause to do so. In the case of adults who display behaviour that is difficult to understand, an intervention-based approach may be adopted. Within this role, you will deal with all sorts of different households and families. Due to the high number of cases involving young people, the Intensive Coordination team comprises, in part, staff from the Centre for Youth and Family (CJG).

Intensive coordination acts as the link between the protection chain (Veilig Thuis, certified institutions, the Child Protection Board and the ‘Toekomsttuin’) and the safety chain (Care and Safety Centre, probation service, police, etc.)

In practical terms, this means that you:

• It provides an overview and insight into the complexity of safety issues through analysis, tailors problem-solving and stabilising factors accordingly, and breaks patterns within the family system and the support services involved.
• You provide intensive support to monitor progress and restore safety, complementing what the resident or family can do themselves. Depending on the nature of the safety risk, you know how to deploy the appropriate tools and expertise. All of this is brought together in a plan for the resident and/or family.
• If a case requires a bespoke approach, you are the point of contact for the WMO and can take the lead in cases that are administratively sensitive, such as when a property is being closed down.

Here’s what we offer you

• A position working 32 to 36 hours per week;
• A salary of at least €3,568 (scale 9) and at most €5,624 (scale 10) for a 36-hour working week. Your actual salary will depend on your experience and qualifications. For this vacancy, we use a starting pay scale (scale 9).
• An Individual Choice Budget (IKB) of 17,45% on top of your salary. You can use this, for example, for:

• Can you work (partly) from home in your role? If so, you will receive a home-working allowance;
• Do you travel to work by public transport? If so, you’ll be fully reimbursed. Do you travel by car, bike or on foot? If so, you’ll receive an allowance for this.

Here’s what you’ll need to bring:

• A relevant higher professional education (HBO) programme, such as HBO Social Work, for example
• At least three years’ relevant work experience, e.g. as an outreach worker, case manager, coordinating support worker or WMO adviser
• Knowledge of clients with multiple problems, families and systems, safety cases and care notifications
• SKJ registration is an advantage
